Discectomy in Iran

What is disc removal surgery?

Between each bone in our spine, there is conjunction called a disc, which absorbs shocks and cushions bones. A herniated disc is a medical complication that, in principle, can occur everywhere on the spine, but it is more frequent for the lower back. Lifting heavyweight or sudden twisting of the spine can result in a herniated disc. However, this complication can be treated by disc removal surgery, also known as discectomy. During the disc removal surgery, the herniated disc that puts pressure on nerve roots and spinal cords, is removed.

Spine Disc Surgery or Discectomy is a type of surgery in which a part of the patient’s intervertebral disc is removed from the spine. Discectomy is usually used to treat herniation or intervertebral disc herniation. In this operation, in order to relieve pressure on the nerve the contents of the torn disc will be removed. The spine consists of the vertebrae attached to each other. It has disks that are elastic and act like a fender and prevent severe stroke to the spine. During surgery, a part of the bone is removed from the problematic disc. The surgeon also examines the spinal nerves that pass through the spinal canal through the neural foramen. Nerve foramina are small openings around the bones of the vertebrae. These nerves have gone from the cartilage to the foramen, one on the left and the other on the right.

Who needs disc removal surgery?

Doctors recommend disc removal surgery for removing pain and bringing back the patient’s regular activity. In particular, doctors recommend the discectomy under conditions as follow:

  • The patient suffers from severe leg pain, weakness, or numbness that prevents the patient from daily activities.
  • Initial symptoms do not fade after at least four weeks of non-surgical treatment methods.
  • Physical examinations and medical tests show that weakness, abnormal feeling, and numbness will be treated with a high success rate after the surgery.

However, if the patient shows cauda equina syndrome, the case will be treated as an emergency, and the patient will undergo disc removal surgery immediately. Symptoms of cauda equina syndrome are as follow:

  • loss of bladder or bowel control
  • weakness in both legs which appears suddenly
  • numbness in legs, buttocks and genital area

Why the patient needs disc removal?

In the herniated disc, a portion of the annular portion weakens and tears, followed by the central gelatinous disc, tearing through the rupture and pressing on the spinal cord and nerve roots.

To solve the problem, the patient must remove this gelatinous part of the body that exerts pressure on the nerves. This is called disk removal or discectomy. However, not all discs are removed in this surgery and only the part that exerts pressure on the nerve is removed.

Types of Spine Disc Surgery

Following are various types of Spine Disc Surgery

Spine Disc Surgery with laminectomy surgery

This procedure is first performed by laminectomy of the vertebrae to determine the nerve root of the spinal cord. The surgeon then removes the gelatinous portion of the disc and then removes all of the gelatinous part of the annulus ring. The surgeon then examines the entire area around the nerve to remove probable patches that apply pressure over to the nerve. The surgeon then extends the foramen, which passes through the nerve root, to prevent pressure on the nerve as it passes through the foramen. The surgeon closes the wound after making sure that the nerve root is completely free and there is no pressure on it.

Microdiscectomy Spine Disc Surgery

The surgical incision in this procedure is shorter and about five centimeters. The surgery is performed with the help of a microscope. The major difference between this surgery and the previous one is that the laminectomy is not done, i.e. the lamina is not removed, but the lamina of the upper and lower vertebrae are removed and the surgeon goes through the spinal canal using special means of space between them, It does before.

The microdiscectomy of Spine Disc Surgery is performed through a much smaller incision. In this procedure, the surgeon will remove the damaged disc by inserting a thin tube with a small camera.

discectomy

Endoscopic Spine Disc Surgery

In this way, an endoscope, a device like an arthroscope, is inserted into the body by making a small incision in the spine. The endoscope is sent through the fluoroscope computer imaging device to the target disk and extracts the contents of its gelatinous core and any part that exerts pressure on the nerve root.

What are the different ways of discectomy?

Disc removal surgery usually gets done in the hospital, and during the procedure, the patient is either under general or partial anesthesia. However, microdiscectomy is another way of doing this procedure. During the microdiscectomy, doctors use a special microscope to check discs and nerves carefully. Using this method, doctors will be able to cut smaller tissues and, therefore, less damage the body.

Disc removal surgery for curing the lumbar herniated disc provides better chances of pain relief than other non-surgical methods.

Treatment methods involving laser beams are rather modern procedures that are less painful and risky. Before the surgery, it is better to consult the proper treatment method with your doctor.

How is a discectomy performed?

Discectomy surgery is usually performed after general anesthesia or regional anesthesia. The patient is placed in the abdomen on the operating bed so that the surgeon can perform surgery on the patient’s waist. The surgeon removes the gelatinous portion of the disc and then removes all of the gelatinous part of the annulus ring. Sometimes pieces of gelatinous disk are completely displaced and pressurized elsewhere.

The surgeon then examines the entire area around the nerve to remove the probable patches of pressure that apply to the nerve. The surgeon then extends the foramen, which passes through the nerve root, to prevent pressure on the nerve as it passes through the foramen.

The surgeon closes the wound after making sure that the nerve root is completely free and there is no pressure on it. Some surgeons place a piece of a special sponge pad or piece of adipose tissue on the nerve root to prevent the nerve from clinging to the surrounding tissue or the extra tissue created after the surgery. These adhesions and excessive tissue pressure can worsen the patient’s problems.

What to expect after the disc removal surgery?

Immediately after surgery and fading of numbness, patients are encouraged to walk and start the physical therapy by low rate activities and exercises. During this time, pain can be controlled with some pain killers and individual prescriptions.

Patients may also experience cases as follow:

  • Not being comfortable while sitting. This happens after the surgery, and it is better for patients to avoid sitting for longer than 20 minutes. This uncomfortable feeling will fade away.
  • Patients need to walk often for several weeks after the surgery. This helps the muscles to gain back their power and also to lower the risk of scare tissues.
  • Usually, patients manage to resume their daily activities soon after the surgery. In some cases, doctors may recommend the rehabilitation program, which includes home exercises and physiotherapy.

Possible Complications of Spine Disc Surgery

This surgery is considered to be a low-risk and less aggressive surgery. However, as with any surgical procedure, it may have complications that are the most likely complications of this operation:

– Postoperative anesthesia problems

– Infection at the surgical site after Spine Disc Surgery

– Nerve injury during Spine Disc Surgery

– Bleeding at surgical area

-cerebrospinal fluid damage

– Damage to blood vessels inside the spine

What are the risks of disc removal surgery?

Similar to other surgical treatments, disc removal surgery has its risks. They include anesthesia syndrome, infection, and damaged spines or nerves.

Postoperative measures of Discectomy Surgery

Most patients are discharged from hospital one day after surgery and can drive after a week or two. One should avoid bending and lifting objects for 4-6 weeks. Usually the patient can do light workouts after 2 to 4 weeks and then go on to heavy workouts and exercise after 2 to 3 months. Many surgeons prescribe physiotherapy after three weeks of Spine Disc Surgery. Physiotherapy is necessary for 6 to 8 weeks after Spine Disc Surgery. It can be said that after 4 months the individual will be fully recovered.

Many surgeons prescribe physiotherapy after three weeks of lumbar discectomy. Physiotherapy is necessary after 6 to 8 weeks after lumbar discectomy. It can be said that after 4 months the individual will be fully recovered.
